Who We Are: Atlas HLTH, operated by TurtleBack Digital LLC, is a behavioral health routing and care coordination platform. We act as a Business Associate to covered health care entities.

How We Use Your Information: We use your protected health information (PHI) to facilitate referrals, coordinate care, conduct clinical assessments, and communicate with your care team. We do not sell your PHI.

Your Rights: You have the right to (1) request access to your PHI, (2) request corrections, (3) request an accounting of disclosures, (4) request restrictions on use and disclosure, (5) receive this notice in paper form, and (6) file a complaint with the U.S. Department of Health and Human Services.

Our Duties: We are required by law to maintain the privacy of your PHI, provide you with this notice, and follow the terms of this notice currently in effect.

Disclosures: We may disclose your PHI for treatment, payment, and health care operations. We will not disclose your PHI for other purposes without your written authorization, except as permitted or required by law.
